GutChek collects certain demographic, health, and health-related information about Users for our Healthcare Providers as part of providing our Services that may be “protected health information” or “PHI” and governed by the Health Insurance Portability and Accountability Act (“HIPAA”).
This information is collected when:
Providing administrative, operational, and other services to a Health Care Provider that is a “Covered Entity” (as defined by HIPAA)
Received from a Health Care Provider when acting as a “Business Associate” (as defined in HIPAA) of the Health Care Provider
HIPAA provides protections for the privacy and security of PHI. Please read the Notice of Privacy Practices of your Healthcare Provider to understand how your PHI can be used and disclosed.
HIPAA does not apply to Personal Data (defined below) that is not PHI. Personal data that a User provides to GutChek when GutChek is not acting as a Business Associate is not PHI. This includes when creating an account, searching for Healthcare Providers, reviews, device IP address, or web analytics by browsing our website.
Any information that identifies you including name, date of birth, profile picture, addresses, phone number, email address, device IP information, fax number will be referred to as “Personal Information” for the purposes of this Policy.
